I can tell you without a doubt that you are her next of kin. My sister passed away recently. Her husband and her had not lived together in 8 years, they had a 21 year old son. All decisions and responsibilities fell on her estranged husband.Thankfully he left everything up to their son and agreed with everything he wanted to do.

As long as you two are still legally married, then, yes, you are her next of kin. As her legal husband you don't have to be added to any medical forms, you're already on them by being married to her.
Should, God forbid, the worst happens, it'll be up to you and other family members to come together and take care of funeral arrangements.
This is all assuming that you two aren't Legally separated. If that's the case, then I'm not sure what to tell you. But it would seem that as long as there's no legal divorce, you'd still be next of kin.

As long as you are still legally married, then yes. If not, then the 'next of kin' falls to the kids, the parents, and the siblings (IIRC, in that order), depending on who is alive (and their respective ages).

You can name anyone as your next of kin but seeing how you are separated and not divorced you are the logical choice
