How can you tell that they are not just trying to sell you their product under the guise that you are not adequately protected. And if you have spyware and virus protection through you ISP, and firewall with Windows, is that really sufficient? If not how much do you need?

I know of one that honestly does it for free: www.housecall.trendmicro.com. They do give honest results. Of course, I've been using their antivirus program for years (no trojans, no worms, no viruses). I'm not sure about other services, though.
As far as how much is enough, it depends on how much protection the program your ISP provides gives. If it only finds some malicious content, then it's not enough. I'm also not sure how reliable it can be (no offense to anyone) relying only on the Windows firewall. Unfortunately, there are still issues out there (as far as I know. I could be wrong!).

It depends on the site.
The ones from the major vendors are usually OK.
Some of the less known brands, I might question their veracity.
I like AVG antivirus and Ewido antispyware both available free at http://free.grisoft.com

Dont bother with websites that "scan" your computer for problems, and say their software can help. There are plenty of free anti-virus progs out there.

For example:
