no people do not have to walk through an x-ray machine. you're carry on items are ran through one though. you have to take off your shoes and place all metal into a basket which they send through the same machine as your carry on items. you physically walk through a metal detector similar to the ones in some high schools. if the alarm goes off you are asked to step to the side and they wand you with a hand held metal detector and possibly frisk you down.

If they made someone walk through a x-ray machine, they could end up with some pretty big law suits. x-raying people too much has been linked to cancer, as well as causing pregnant woman to have deformd babies, and miss carry. It would be a huge liability.
Instead they use metal detectors, and if that is not enough, they use hand held metal detectors to determin what is setting off the machine and pat down that area. If that is not enough they can take you into the undressing area.

I don't know, an Xray machine is pretty hard to throw. They do have to walk through a metal detector and the luggage gets scanned.
