There is a lack of consensus among fans regarding whether or not anyone actually understands Stewie (the question is even brought up in the "E Peterbus Unum" episode), but MacFarlane stated in commentary that he and his staff have decided to never set a rule about it, saying whether or not the other characters understand him will depend entirely upon what the writers feel like doing at the time. It is generally accepted by fans that though his parents understand Stewie, they write off most of his talk of murder, world domination and the like to be the imagination of a baby. One of the show's writers commented on a DVD that the reactions to Stewie's insults are "like if a 4 year old came up to you and said '**** you', you'd just laugh it off and think nothing of it." Brian (the dog) is the only one in the show who can hear him 100% of the time and actually takes him seriously, so they often end up pairing off comedically.

Right, so Brian the dog can understand Stewie, but no one else in the family. Yet the family understand Brian when he talks and he's a dog?! Doesn't matter the show is hilarious!!!

yes he realy talks and they can hear him cuz in 1 on the episoeds hes talking 2 som1 they r like r u a talking infant?they can also here brian the dog bcuz som1 asked the same thing they did 2 stewie.

I think they just hear baby talk, and maybe words like "mommy" and I think Lois can understand when he wants something, just like any other mother understands what her child wants even if they don't use words. I also agree that Brian can hear everything full on.

Good question, I thought only the dog could understand Stewie.
Then in certain shows others talk back to him or appear to understand his jabbering.

thats something i questioned myself, actually, the only one that can understand him is brian, brian can full on hear and understand him and he knows that he's some evil little kid that hates his mom, the others can hear him talk but they dont care, seth mcfarlen said this himself, well its not that they dont care its just, they dont notice.
