In my studies all I could find were the note books. Visitors would write their questions and Beethoven would speak and tell them the answer. He didn't have to write the answer because he could speak. His household probably used common sense signs for eating sleeping etc.
Isn't the sign language we speak of American Sign language. As Bethoven was in Germany our sign language might not help him.

I dont think beethoven did know sign language. I think he relied on vibrations. and possibly reading lips. he also wasnt deaf from the beginning. he didnt lose his hearing till later on in life if my memory is serving me correctly.

Sign Language did exsist back then, sign language was the way people communicated before there were words!
