If you can find some thick wedges of foam rubber at a fabric or upholstery shop glue them together into a round shape and carve out the "core" so you will fit inside. Then, use an electric knife to "carve" the circle into an apple shape then spray paint red....there are spray paints available at craft stores that should be compatible with foam. Carve a hat out of the same foam in the shape of a piece of the core and just paint the top. Glue some black felt seeds to the core. Cut a big green leaf out of felt and attach to the top edge of the core cap. Make strings that will tie under your chin to keep the hat on. Wear red or green tights under the apple part of the costume.If you can't find the right spray paint you could glue red felt to the foam apple to cover and leave one wedge white as if the apple had been cut..glue black felt seeds in it.

You can make a sandwich sign apple costume and wear red leotards to go with the apple sandwich

you could get a pattern from the cloth store...
if they don't have apples (which they may not), they usually have a pattern in the books for a baseball or a pumpkin that you could adapt to an apple.

I made my kids tomato costumes one year (Attack of the Killer Tomatoes!) using red bean bags! worked great.

Your best bet would probably be either paper mache' or styrofoam.
BUT you could also make some kind of apple shaped "shirt" that is very large and that can be stuffed. Then you would wear red tights, make an apple top hat and you're ready to go!

My mom made a pumpkin costume one year. She took wire coat-hangers and straightened them out. Then made 2 small circles (one for head, one for legs). Then bow several more hangers in an arch. Attach each end to the circles. Then cover with red paper. Wear a green ski cap for the leaf/stem.
