That's like asking what makes a cake a bad cake. A million different things can go wrong.
It's making things go right that narrows the choices. But then there's an artistry to making a really good cake (or movie), and you can't quantify art.
But if you insist on a list of essentials, start with writing, directing, cinematography, and acting. If EVEN ONE of those things is bad, the movie will be bad.

Well, there's poor acting, unbelievable storylines, etc., but I discovered an empirical test when I was taken to see a French film and needed to go for a pee-pee part-way through. The empirical test for a bad movie (for me) is that when you're already some way into it you have absolutely no idea how much longer it's going to go on for ... and so have no idea whether to tell your bladder to be patient, it's only five more minutes, or whether to work out a way of piddling yourself when in the middle of a row at a movie theatre without drawing the attention of the people either side of you.

It could be many different things. In some cases, it is bad acting. Sometimes it's bad writing. Or it could be a good story and good acting but the director has a take on the story that really does not work.

When the actors are not believable, the dialogue is ridiculous, the plot goes nowhere. Stuff like that.
