LOL.... I'm not sure if it's a rumor but I heard he got it from Bolo Yeung when they filmed Bloodsport back in '88. Probably a mishap while filming a fight scene or something. If you pay attention to the movie, Van Damme doesn't have his bump at that time. Fact or fiction? Not sure...

I believe he got the scar/lump as a kid. In an old interview he said he was in a swimming pool, came up from under the water and his head was under the springboard just as someone bounced off it. Head comes up as board goes down:Ouch!
It's great that in all his "twins" films, both brothers have this lump (and dodgy accents).
