This is a really hard question to answer. But my friend is going through the same thing right now. And what I told her was that you can't stay with someone if you are unhappie because you are scared they will hurt themself. Just stay in their life be friends with them. And if you are sure they will hurt themself have someone that is with them 24/7 to keep an eye on them once you and him break up. Sorry I can't be of more help!

If they have been making threats, call 911 and get them committed. If it's just a feeling that you have and they haven't made statements like that, you need to talk to a professional and see if your fears are reasonable. Either way, you shouldn't stay with the person, at least not once there is someone there to look out for them. The other person's safety is not your responsibility and the best thing you can do is get them some real professional help.

No. That situation could go on indefinitely. You'd want to end the relationship at some point, right? Also, if the other person is aware that he/ she's doing it, it's emotional blackmail and pretty low.
