trying to get a passport for my 5 year old daughter, it requires the signature of both parents since she is under 14 years. her father has been out of our lives for along time. how can I get a passport so that we can go on vacation out of the country?

There is a statement of consent form (DS-3053).

In part five of that form SPECIAL CIRCUMSTANCES, you note that the father have been out of the child's life for the entire time, you do not know where he is and you cannot contact him.

Then you sign off on it.

If you have sole custody of her, you do not need her fathers signature. You could talk to an attorney and see what they have to say. Most do not charge for a consultation.
Do you live in the US?
When I got passports for my kids last year for a trip to Italy. I didn't need to have my husband sign. They were fine with just my signature. And my kids are 3,8 & 10

go to the friend of the court and tell them that you want to go on vacation out of then country adn the pass port requires both signatures..let them know that the biological father is no where around and no where to be found and in most cases the friend of the court will aloow you to go for this vacation

Contact a laywer for a possible court-ordered Unable to Sign allowance. Otherwise, just mail it to your husband.

If you have child support it will be enough to issue the passport.

forge his signature , if u cant or dont want to just turn it in , theres suppose to be a divorced status form in there .

you dont without sole custody papers signed by a judge or a death certificuit signed by a doctor
