The titles were given by the officials in the Roman Catholic Church over many centuries. Usually, a title came about because of some question about the faith and Mary's place in the whole story.
After 2,000 years of questions, it isn't surprising that she has so many titles.

yep, Irongrama is right, that is what I wanted to write as well. Maybe I can add that this took place after the Constantinian Conciles (around the year 335), when they started pushing harder. At that time (until about 1100) there were three religions popular in Europe: the Midas cult from Persia, the Isis Cult from Egypt, and the Jesus Cult from Palestine. You know how it ended.
They re-wrote the Midas Lifestory and put it into the bible (from then on it was the life of Jesus being told), and they wove in several goddesses from several times and cultures to satisfy the common people in their need for a female deity. (As Irongrama already mentioned, people were used to female deities and were very reluctant to this new religion from the palestine area)
Actually, Queen of Heavens was originally Ishtar's title, you can still find that chapter in the Bible (Old Testament).

When the catholic church began emphasing Mary they were trying to eliminate the remnants of goddess worship. They were having a hard time getting the people to accept their masculine dominated religeon, completely eliminating the female from worship. To win the people over to monotheistic, paternal religeon they had to throw in a woman. Mary the mother was considered easier to accept than Mary the wife, so Mary became Queen of Heaven among all of the other titles.
