Unfortunately accidents happen, I would still go to the dentist, scared or not. There could of been many reasons she died, it may have even been some ailment she already had that the sedation may have provoked.

Please understand that she had intravenous sedation AND oral sedation AND inhalation sedation (nitrous oxide). I do not know the circumstances, but please do not think that this is suddenly a common occurence. Even with this combination of drugs, this is rare.
I am not defending the guy, because I know nothing about it, but this seems like a lot of sedation to beused in a private office setting.
If this stops you from going to the dentist, how are you ever going to get in a car again? Aren't there still about 50,000 people killed in cars every year?

Most children don't need sedation, My children didn't have any to fix thier surface cavities.
Then my retorical question would be... Would you not get treatment from a Dr if you knew how many people die every day from medications or in the hospital?

Anything is a risk, even minor surgeries. There was a young boy here who died after getting his tonsils out. That is a fairly common procedure. You never know how the body will react to things. It's sad.

Oh for pity's sake! Would you get behind the wheel of a car knowing that everyday 100's are killed in automobile accidents? Did you know that you could slip on a bar of soap in the shower and fall and die of a broken neck?
