Soemtimes its a shorten name given a parent calls a child.
ie Meg instead of Meghan. The person can usually decide to be called that nickname or want to be called by his or her orginal name.

ie my brother RJ (Richard John) doesn't mind be called Rj while my sister Kitty (Catheryn) will get offended if osmeone outside our immediate family calls her anything but Catheryn.

Sometimes it's a name given you to by a friend.
ie My best friend Krisitn is called Toussaint, mainly because that's what I started to call her. But her parents don't call her toussaint, nor do other circle of friends.

I guess the best example would be my nickname.
Meghan- Called Meg or Meggy by family. Meg by my friends. But I intorduce myself as Meghan and write on my Papers my name is Meghan, not Meg. I don't mind being called Meg by starngers, but if I don't like you andd you call me Meg I will tell you it's Meghan.

Usually it is given to you by someone your know eg relative or friend etc. Sometimes it is a shortened version of your own name, or a description of something about you. All of my sisters have a nickname. I think it was my parents cruel gift to us. The are George, clodhopper, jellybean, tugboat, & guppy. I won't share which lucky name I was given.
