How many for severe pain and how often?

what does the label say???? I would say 4 every 6 hours, my husband is about your size and thats what he takes

Follow the instructions on the label written by your doctor. If he/she felt the dosage should have been different it would have been written differently. If you don't receive adequate pain relief in a day of so call him and let him know.

When I had a badly damaged shoulder my doctor put me the 7.5mg 1-2 tablets every 4-6 hours as needed. And at the time I weighed 180 pounds.

Now, if you are asking and this is for recreational use....1st...don't. OK, having cleared my conscious and CYA'd myself I have know people that have taken 5 or 6 at a time just to get high. But is the rapid development to tolerance and the possible withdrawal worth it?

Really, the body tolerates a lot of synthetic morphine (hydrocodone) I would say to start you are safe with 2, no more than 3. You will quickly develop a tolerance and can take more (over the course of a week or two). Watch out for addiction, its easy to get dependent. Your greatest risk is from liver damage from the acetominiphen (the 500 part of the name).
