my ex husband has the original and i do not know where he lives now, nor how to get ahold of him. would i get it from the hospital where he died? or ?

Your state probably has a Bureau of Vital Statistics. If you cannot locate such a department in your state, call your county clerk who may be able to answer. In some states, it is the State Health Department who handles such a thing. Good luck.

You can get it down at the courthouse in the city where he died. It's the same place you would get a marriage or birth certificate.

I would go down to the court house into register of deeds they would have it in the county he died in.
