With back pain and knee pain you can't do a whole lot. Swimming would be excellent but you said it has to be in the home.

If you must stay at home, the only thing I can think of is rebounding. Rebounding, in case you haven't heard of it, is a small trampoline. Good ones for beginners will have a handle to keep you balance when you first get started. It is very little impact and a great workout....like jumping rope without the impact.

Here's a link to a good one for just $60.00...not too expensive as the shipping is probably free.

I have a herniated disk in my back so I have to see a chiropractor once a week. Here are some of the excersices he has me doing and they work really well. First lie on your back on the floor next to your sofa scoot your behind all the way up to the couch laying your feet on the couchand then do crunches this way it helps to keep your back straight and less tension in the back while doing them he got me up to 100 a day and my back feels great! you can also do some leg lifts and stretches on the floor as well as the scissors where you lie flat and bring your legs up and out those are just a couple of things that work for me but they make really good instructional dvds now for low impact excersices good luck

Pilates is great for toning. If you want more cardio then swimming is good or dancing. Denise Austin also has a great selection for low impact aerobics.

Yoga, or Pilates. Go out and buy an interesting looking video/DVD. A body ball (giant rubber ball) will help you stretch out your back.
