It has many meanings, depending on how it was used and by whom to whom.
In some cultures, it is the sign of the devil. In Texas, it is a sign of support for a college football - the Texas Longhorns. In sign language, it is a fist with the thumb extended, the forefinger extended and the pinky finger extended - and means "I love you."
In a trip to Texas, President Bush made headlines across the globe when he held up his hand as you depicted - with only the forefinger and pinky extended. He was saluting the Texas Longhorn football team, but in other parts of the world, the photo indicated he was saluting the devil.
That's what happens when one gesture can mean different things in different parts of the world.

Its a sign of the devil, but metalheads use it at concerts when a band is really ace... instead of pumping yr fist in the air, some people do the sign of the devil... it's spread from the metal scene and I've seen heaps of people at rock concerts doing it too... I've done it once or twice myself.

In some places, it's a charm against the evil eye called the "mano cornuto" or the horned hand. Depending on who you talk to, it's meant to be the horns of the devil or the horns of a goat.

R U sure that the thumb is not sticking out also. Because that's how you say I Love You in sign language I taught it to my kids and we always said it when we wanted to say it silently. Hold out your thumb, pointer finger and pinky.point to the 1 u love.
