People, people, people. Do you think they needed injection molded plastics to create a toothbrush or something? You can find millions of references on the history of oral hygiene. A very easy and general discussion in found at http://www.oralb.com/learningcenter/teaching/history.asp As long as there were civilized societies, people cared what their breath smelled like. They probably were even more interested in tongue scraping than modern society.
It's a good point that people didn't live nearly as long as we do today, so there was probably less dental disease to deal with. It's probably also valid to assume that the diet of a few hundred years ago (and earlier) was not so hard on teeth, due to the lack of refined carbohydrates.

It was poor, at best. There wasn't much thought to dental care, and lots of people lost their teeth.

The average life expectancy was less than 50 years.... even the worst cared for teeth probably could last that long.

people did not have to brush their teeth back then. teeth would stay good for a lifetime. lifespan was normally 30 to 50

mostly nothing, but some people did chew a stick until it was frayed and then rub wood ash on their teeth , it did help a little.
