My understanding is that anyone who has a part in the ceremony and was at the rehearsal is invited to the rehearsal dinner. Sometimes the invitation extends to spouses or significant others, but not always. If Grandma was at the rehearsal because she is going to be formally seated before the processions, then she goes to the dinner.

Now, I have seen rehearsal dinners that were more like family reunions, so I guess it's up to whoever is paying.

Yes, if they are part of the wedding party.......but I personally would still invited them if they were not in the wedding party.

If they are in from out of town or part of the wedding party, then yes. Otherwise it is a personal choice.
