I am sure the number is in the millions. But because smallpox has been around so long and so many died without treatment, it really is impossible to say exactly how many.

I don't think there is a way to accurately quantify your answer as a number due to lack of data over the centuries, but the fatality rate was in the 20-40% range.

In the 20th century after data was available, per the WHO it was 300-500 million deaths before eradicated by vaccine.

Millions, as smallpox has a high fatality rate, and a vaccine was not in widespread use until the 18th century.
