The name comes from a series of 12 paperback books called "The Fundamentals", published between 1909 and 1915. Each volume contained 7 or 8 essays on various topics, which supposedly covered all the fundamentals of the Christian faith. 3 million copies were distributed free to Protestant preachers and pastors, and the Fundamentalist movement was on its way.

I don't know of any particular book but I know that American Protestants were the first to use the word "Fundamentalist" in the early 20th century to distinguish themselves from the more liberal Protestants they believed were distorting Christianity. I hope this helps, I'm sorry I don't know of any book.
