Find an old picture of the two of you and then take a new picture of the two of you, then frame them side by side so he can see how youv'e both grown as a couple.
Or buy him a gift basket of his favorite candies, since he has everything food or candy may be a good gift because he can eat them and then that's it.
Or if there's a special book he might like...a new gadget for the kitchen or garage.
Maybe clothing if worse comes to worse, men always could use some new shoes or luggage. Good luck!

Make up a little "booklet of vouchers" on your computer. Each voucher is good for one thing. For example:
Page 1. This voucher is good for a two hour message (or do 2-one hour messages)
Page 2 Good for one Breakfast in Bed at any time
Page 3. free pass for a night out w/ the guys.
page 4. "get out of Jail or the Dog House" free pass
Page 5. Good for one home cooked meal of your choice (must be submitted in advance)
Page 6. Good for one (Insert your own intimate/sexual details here)
Page 7. Good for one dinner at the restaurant of your choice
The list can go on and on. Be creative, have fun with it, and Congratulations!

that depends does he have any hobbies, or is he into sports? if so get him ticket to a game that he likes. or you could ask his friends they would know and that way you get a mans point of view. maybe dinner and a movie that's a bit more traditional. or you could plan a trip to somewhere he's always wanted to go or has already been. you could plan a quiet evening at home and hire a chef to cook for you. have a party. you don't really have to give him anything, memories are a great gift to give. happy anniversary!!

His slippers, a beer and a lovely surprise meal? Nah! Only joking!
Actually, your question implies that you DON'T have everything you have ever needed ... now just what do you mean by that?
Maybe your question should be rephrased: "What should a wife expect from a husband who has everything after 21 years of marriage?"
You tell us!

a sky-diving gift certificate.
And get yourself his signature on a new life insurance policy.
Ok, that was silly.
IT'S HARD, isnt it? How about just a nice date? We take for granted "dating", so how about you two go out to a movie, have a burger afterwards and then come home and snuggle with a bottle of pinot noir?

What about preparing his favorite meal, doing something that is special to him, making a card for him that is specific only to him, thank-you........for our beautiful children, your great attitude, how you make me feel when I am with you, thank-you for being a great husband/father/person/grandson/brother/provider/ etc. The possibilities are endless, most times it's the little things that are appreciated most. I find among my friends, that they like specific thank-yous, a favorite meal, or a small party with his favorite people included.

Reminds me of a funny story I once heard someone tell-
The man said that his wife gave him a pack of cheap screwdrivers as an anniversary present. He said, "What's this for?" his wife said, "This is for 20 years of horrible sex" So in return he gave his wife a frying pan. She said, "what is this for?" and he said, "This is for knowing the difference."
