Yes just ask your GP and they should supply copies but they might charge £50.00

That depends on how old you are. There is a statue of limitations, meaning a doctor or healthcare facility is only required by law to keep records for X amount of years, Im sure that is different in every state. Charges for them may vary, but if they still have them you are entitled to a copy.

Sure childhood doc....or if anything more serious try your hospital...thats if you know which one you would have been treated in..

yes your pediartrician should atleast have them on microfish
