Wally Lamb's "I Know This Much Is True"
its about 2 identical twin boys, them growing up, then one getting diagnosed with schizophrenia. its a great book, even though its quite a long story, i couldn't put it down.
i also enjoyed Wally Lamb's other book "She's Come Undone"

The most exciting fiction book I have read for every year is Crime and Punishment. The "best" is a toss up between Crime and Punishment, War and Peace, and The Brothers Karamazov. If you see a theme in my choices you are right.
