tell the they irritate you while you are smiling. if you don't express verbally, they will continue their behaviour.

Depends on how long you think you can keep smiling. It's probably better to nicely tell them that they irritate you, but be prepared that maybe they will get mad or have hurt feelings.

It depends on the relationship. If it's your boss, best to just smile. and if it's someone who could stalk you, best to just smile. If you have some control, find some funny way to let them know you're not interested.
