I've seen these adult pictures of men with unusually large organs, several times larger than anything I've ever encountered. Could they possibly be real? What would cause such a thing? Is it a medical condition? I don't know how I'd react if I ever saw one in real life!

For the most part, they are real.
Male performers are chosen principally for their over-the-top size of their... members! In fact, the standards have reduced since the 80s, where big boys like John Holmes, Ron Jeremy, and Dick Rambone were there monstrously endowed beefmen, the likes to scare the most die-hard hardcore addicts. But as it is the case in most of these "actors", size has a price to pay, often resulting in the member in question barely being able to perform.
Which reminds me of my own philosophy: what's the point of being so big if you can't get it hard?
These days, although most males in productions are in fact well-endowed, they aren't montrously so, making it easier for them to perform on cue.
And yes, I have seen some of them up close & personnal, being in the adult biz myself as well... However do note that alot of productions these days use wide angle lenses for closeups, and also shoot from POV and other angles, often making the tolls look larger than life.
But in the case of most of these guys you see perform, it is all real. The only real "operation"/surgery that some go through is not for enhancement of size, but rather for ejaculatory enhancement. But that's another story for another question, isn't it?

I think they are not real. They must be using some covers otherwise it is not possible to have such big penises .For ladies,I can say it is possible to have big boobs by surgery or hormone therapy but for penis enlargement ,it is not possible .The penis size is determined by genes inherited from parents.

it's real but not normal, and not good either
imagen the pain u will have if u were makin love with a guy that has a 12in long and 8in round penis!!!
and imagen the pain they have when wearing pants, specially swimin trunks...... and when they sleep and have to lay on the side......

Hi. I am afraid it is real. Some guys are just born that way. Just like some women have gigantic breasts. Amazing, huh? But kind of scary if you ask me.

yes, they are real but not the norm... just like women. some have extremely large breasts. that look too big to be real.. they are both the exception ,

it's genetic and, yeah, it could be real; it's just a combination of the right genes, the right nutrition and hormones kicking in at the right stage in development
