Try hairspray (pump or aerosol) for the ink. Douse the stain with the hairspray & then rub hard with a bar of soap (bath or laundry). Rinse in cold water (Hot water can set non-greasy stains). Wash as usual.
You can also try rubbing alcohol to lift the ink.
On colored items, you can soak the garment in milk. They say it works. On white fabrics, rub the area with a mix of lemon juice and salt. Hang to dry in the sun.
For the set in stains, mix a diluted solution of vinegar, soap and water. Dampen a cloth with this solution and press firmly against the stain for several moments. Blot dry with a a rinse cloth.
You can also mix equal parts salt and baking soda, add several drops of white vinegar and enough water to make a paste. Check carpet for color fastness, then apply the paste to the stain and let it set. When it's dry, brush it away and the stain should go with it. For a particulary nasty stain, scrub it when you first apply the paste.
Hope this helps!

Put item of clothing on a piece of paper towel.
Just squeeze some of the dishwasher detergent on to the clothing. Make it thick enough so you can't see the stain through the gel.
Use a clean dry face cloth, or a dish cloth, and gently rub the stain, working the gel around until it almosts disappears. By this time, you should be seeing the stain starting to disappear.
If it is still there, put a little more gel on it and rub with your cloth again. It will come out this time. If it works the way it did for me you won't even know there was a pen mark there.
Make sure the Dishwashing detergent doesn't have bleach in it if this is a colored shirt though!
