How high depends on lots of things -- how much the balloon material weighs, how much it's filled up, weather conditions and winds, etc. Basically, it will rise until either the air pressure outside the balloon is low enough for the balloon to pop (remember, the higher you go the less air pressure there is), or until it gets stuck in a thermal inversion layer, or until the balloon leaks out its helium (balloons never have a perfect seal). What usually happens is they rise, the air pressure outside decreases letting more helium escape from the balloon, and eventually enough leaks out that it begins to sink from the weight, eventually bringing it back to the ground. Five or six thousand feet up isn't out of the question before this happens, but much higher is unlikely.

depends on how full it was filled... if it was very full it may get high enough to pop... if not, it will rise until it reaches a point of equilibrium with the reduced air pressure around it and will follow the air currents... but then it will start to lose helium and that equilibrium point will get lower and lower until it reaches the ground again....

They rise up and up for a while, but do eventually come back down to Earth. Blue Peter sent some up with cards on and you had to phone the number. It was to see how far they travelled. Just tried a search for it, but couldn't find it, so sorry, no link!

when i was little i ued to get 3 or 4 balloons and tie them together and tie a ball of wool to them and let them fly, i think my record was 4 balls of wool,
may be irrelevant, but it was fun
