Easiest thing I can suggest is to write the document with one column initially, then select the text that needs to be in two columns, and then make only that section into two columns.
Other options include:
(1) Creating a text box to hold the header. Put it where you want it.
(2) Setup the document in a 2x2 table with the header in the top row spanning both columns and the rest of your text in the other two columns.

Begin by typing the title and get it all spaced and formated the way you want it.
Then leave two blanks lines below the title
Select Insert then Break, and in Section Break select Continuous
This will place a break between the title and the rest of the page
You can now do a Format and create your two columns, but it will only create them below the section break. The title, which is above the break, will not be affected.

I think...Hold down your pointer on the Tile, drag to the right 'til it stretches over to B column or C, D column, for however long the title is, hope this helps!

create your table the usual way, with the 2 columns and however many rows you want:
· table > insert > table
then:
· highlight the row you want the title in
· right click
· choose 'merge cells'.
by doing this, the 2 columns on that row will be merged into one cell which spans the entire width of the table.
