Due to a change in personal circumstances i must request a reduction in my contracted hours.

If any of your children are five years old or under, or under 18 and disabled, then as a parent you have the right to request flexible working in order to take care of your kids (Flexible Working Regulations 2002, inserted into the Employment Rights Act 1996, Ss.47E, 80F-I and 104C).
Your employer should have a policy on flexible working. Ask HR for a copy. You will be asked to fill in a form and attend a meeting. You should think carefully about what you need, i.e. reduced hours, late start / early finish, part time, job share, and be prepared to explain to your employer how your request can be accomodated without undue expense to the business.
Your employer has a duty to consider your request. However, they are not bound to agree to it. They can refuse on a number of business grounds such as burden of additional costs, inability to reorganise work amongst existing staff and detrimental impact on quality.
These rights apply to England, Wales and Scotland only. Similar rights apply in Northern Ireland, as they are all based around the same EC legislation, but I am not familiar with NI law.

Is there knowone you can have a chat with face to face? It isn't always neccessary to put your intentions or requests in writing. You can ask your management personally and then go about changing it in your contract if they agree. The only reason I would say you definitely should put it in writing is if you think they may make an issue of it somehow and then you can have a history of how you put forward your request (you need to keep a copy of your letter for this of course).

Sir, I am writing to you because my circumstances have recently changed and I wonder if you could accommodate a change to my working hours ?

Maybe phrase it along the lines of:
Due to unforeseen circumstances within my personal life which are beyond my control I urgently need to reduce my working hours - obviously my work load will not suffer, and where possible I will carry on with what work I can from home.
I am sure that you will empathise with my situation - which I am available to talk to you at any time if you so require.
that kinda thing - any understanding person will make allowances for ya! Good luck

Be very specific about what you want:
Limited hours, telecommuting, job sharing, benefits, work only during school hours, etc.
With your wife, discuss what the optimal situation will be and then together make a list.
Add that to this request and we can take it from there.
