I never did. I mean I never leaked it but I could squeeze it out at around 5 months.

I started at about 5 months with just drips once in a while, and then it started to be that i had to wear a tight top to bed cause i could feel it trickling at night. I am now 36 weeks and have had to buy those bra inserts otherwise (one in particular) leaks right through a lined bra. Mine leak more when the baby is really active in my belly.

I was late in my third trimester and it never actually leaked, I would have to squeeze my nipple a bit to get it to drip out.
Kind of like when you do a breast self exam. Rub in circular motion all around the breast and feel the nipple arieola area with a slight squeeze. Every exam during the last few weeks would produce a few drops.

Depends on the person. It's normal to start leaking between the fourth and sixth month of pregnancy. My first pregnancy, I didn't leak at all.
