I was notified by Social Security about my pension due but can not find the Pension Fund address to apply for my claim?

Call your former employer, ask for Personnel and get the info from them since they supplied the pension fund in the first place.

Check the Link, the address shown is the same you posted, but there is a phone number that might still be good that you can call.
Aparently there are several plans operated from that address.
