You need to have good qualifications, firstly. My aunt has been a nanny for the kids of such people as Julia Roberts and Reese Witherspoon.. and lots more. However, she's done childcare in University and she's practically raised all the kids I know.. there are over 100 kids who she'd call 'her kids' although she doesn't have any. Reese Witherspoon, I know, was particularly impressed with her qualifications and had her look after the baby for much longer than was necessary.
So I guess what it really is is lots of experience, good qualifications, good interviewing skills (as they have interviews to choose who they think the best nanny would be).. and alot of patience and looking around!
