5. The teacher was respectful to all students.

I'd also leave some open-ended questions (what did you like most/least about the course?) or places for additional comments.

Hmmm...I just re-read your question and wonder whether you were asking from an administrator's perspective rather than the student's. In that case, I guess I'd have slightly different criteria, such as keeping up with professional certifications, ability to utilize different teaching styles to reach all learners, classroom management, relationships with parents and colleagues, maintaining a professional demeanor, strong work ethic, represents the school well in other settings...that sort of thing.
