Is this even done any more? You might ask a professional how it is done. I think it would be a lot of work for a one night joke.
I think its done using a skirted table with a hole in it. The fake cake, made of Styrofoam forms or card board or plywood is covered with whipped cream (frosting is v. heavy ) and the girl climbs out using a step ladder hidden under the skirted table. I may not be making my self clear. The cake only needs to be a few layers high by this method and the top layer can be fitted for a hat.
But it's a lot of work for a one night joke and unless you plan to make extra money by taking this show on the road, I dunno.

I believe those cakes are cardboard with tissue paper as frosting.
First you would have to find the round cardboard cyliders possibly a local runnings store or some fleet store may carry them? You may beable to find one that just needs to be put together that you can order online?
If you can find round cyliders try to find three different sizes so you can stack them on to each other then be creative with decorating as you would with a float.

Build a round frame work, and coat it with icing. Cover with large mesh screen to hold icing. Or forget icing and make a faux cake with materials from a craft store.

Use cardboard box, you can buy them at those postal stores. A cheap way to make an icing is to use laundry powder, add water and color you want and whip it with mixer, it will harden like real cake icing. Use your imagination, you will be able to pull it off

take a cardboard box, tip it upside down, and decorate it like a cake using frosting, etc. You can sit (or stand depending on the size of the box) under it and push out through the top. Have fun!!

Those cakes I've seen where people jump out of them, they're usually fake. Sometimes they add a layer of real cake on top of it but under that its all fake.

Your friend should realize that a guy usually doesn't want to see his own girlfriend jump out of a cake.
