esters are synthesised by a reaction of an alcohol with and acid thsat is the simplist form.
there are too many different ways of forming tyhem for to even strat typing out equations but heres a few good references. look at section 4 on wikipedea and theres a list of all ways they can be syntheseised http://en.wikipedia.org/wiki/Ester.

easiest way would be for you to open up a textbook and read about esters.

To go at any type of reasonable speed a catalyst needs to be present normally H+.
