Where they caused because the users abused their prescription and took more than they should have? Are doctors too quick to give out prescriptions without deciding if the patient really needs it? Have there been deaths from this even when a patient took the amount they were supposed to?

Because the FDA doesnt care about people only money. The might test a drug for a month or 6months on a person & discover small reactions, but not know the long term effects until consumers have used it for years. I have also heard that congress members are allowed to invest stock in these companies, so they pass the ones that they feel they are going to cash in on & dont really care that it may kill people down the road.

There are no deaths or very few due to prescription drugs that would make prescription drugs a known danger, there are howerver many many cases of organ failure coincidently while taking prescription drugs.

blaming the organ takes the heat off at least in this life hehehhheh

all of the above, everyone has a different body chemistry, some will take too much, some are very sensative, often doctors offer a pill just to get you out of their hair, and they cant possibly test enough people to know that one in a million will die from the stuff. medicine doesnt work the same on everyone and lots of time they take it with other drugs the doctor doesnt know about , including booze.
