Not far into the ear canal is the ear drum, if perforated with the swab this can result in deafness, the box is only stating the obvious, that you use caution when utilizing the product. Some people suffer from over sensitive ears an they may prove itchy, your pinkey should solve this problem, it shouldn't go too far, however you may wish to try an ear wash through the doctor or purchase Hopi ear candles which I strongly recommend to remove build up, and perhaps a sorbolene based moisturizer for outer ear application.

IF you can see it-then you can try removing it gently with tweezers-but only try once-if you are unsuccesful at removing it then make an appt asap to see your dr or go to the nearest urgent care.
the ear canal itself is only two inches in length and if you try to "fish" for the Qtip (blindly putting things into the ear canal trying to remove it), you could push the tip further inside your ear which could cause injury to your eardrum or could trap germs inside, causing infection.
do not try to put any liquids inside your ear to help remove the Qtip-this could have a reverse effect, causing the tip to slide further into the ear and being that the cotton will absorb the liquid you used-once deep inside the ear canal, it provides the perfect warm, moist, dark environment for germs to breed that cause infection.
the dr will be able to safely remove the Qtip w/out causing further damage and it only takes about 2 minutes

have someone help you. Grab a flash light and some tweezers. YOu hold your ear from the top part and pull and back. your helper will CAREFULLY reach for the cotton tip with tweezers. BE VERY CAREFUL

my son had the same problem i took him to the doctor and they tried to flush it out with this tool i think it was called an elephant ear well needless to say that didn't work so i had to take him to an ear specialist and he he put some kind of sucking device in his ear and it worked the doctor informed me that we really shouldn't use q-tips to dig in our ears he said we should take a clean towel and just wash our ears while in the shower call your doctor 2morrow and make an appointment that's nothing to play with

Try another Q-Tip (gently, and making sure that this one's going to stay attached). Failing that, try rinsing it out in the shower.
Next time, nothing smaller than your elbow, okay?
