I would say it's a question of degree. If you are susceptible to something, it will probably affect you. But if you are vulnerable, it certainly Will affect you , and the effect will be negative or harmful.

Since this section is related to infectious diseases, I'll address it as such. Susceptibility, in reference to drugs may address whether a particular drug will work in a specific situation. Example: Seen in resistance testing with HIV, most people should have a virus that is susceptible to a drug-meaning that if given that drug, there should be a positive result-the drug should work. The opposite is that the virus is resistant-if the same drug is used-it will not work because the virus is resistant to that drug.
Vulnerability or being vulnerable lends itself to a weakened state. Example: with advanced HIV disease or AIDS, since the immune system is not working at an optimum level, the body is left vulnerable to infections and/or conditions that the immune system cannot contain, kill or kick out. Invulnerable is not used as the opposite-immune would likely be used.
Outside of medicine/infectious diseases, the meanings would be similar though not exact.

In physics, the susceptibility of a material or substance describes its response to an applied field.
Vulnerability is the susceptibility to physical or emotional injury or attack. It also means to have one's guard down, open to censure or criticism; assailable. Vulnerability refers to a person's state of being liable to succumb, as to persuasion or temptation.

They are very similar. But vulnerability indicates that someone is almost a victim, that something due to their own weakness induces an effect. Susceptibility seems to just mean that such and such could happen.

Susceptibility is more about possibility of occurrence of something unwanted being known as fact, while vulnerability is about a weakness in defence or lack of it.

There are two types of herpes simplex virus: HSV-1 and HSV-2. Both virus types can cause sores around the mouth (herpes labialis) and on the genitals (genital herpes). Cold sores are caused by the herpes simplex virus (HSV).
Cold sores sometimes called fever blisters, are groups of small blisters on the lip and around the mouth. The skin around the blisters is often red, swollen, and sore. The blisters may break open, leak a clear fluid, and then scab over after a few days. They usually heal in several days to 2 weeks.
The herpes simplex virus usually enters the body through a break in the skin around or inside the mouth. It is usually spread when a person touches a cold sore or touches infected fluid—such as from sharing eating utensils or razors, kissing an infected person, or touching that person's saliva. A parent who has a cold sore often spreads the infection to his or her child in this way. Cold sores can also be spread to other areas of the body.
