HELP! I''m BLOCKED!! I need to know steps to do this:
This is a mcg/kg/min Q:
Order: Med "X" 450 mg IV per 500 mL NS at 4 mcg/mg/min? WT= 198 lbs.
What is the Rate in mL/min? & What do you give in mcg/min?

I meant 4 mcg/kg/min.

Hi,
4mcg/kg/min means that the patient should recieve 4mcg of medicine per Kg body wt in 1 min. Since the body wt here is 198 lbs. (About 89.8 Kgs) so the patient should recieve 4 X 89.8 = 359.2mcg or 0.359mg of medicine/min.
First mix the 450mg of Med "X" in 500mL of NS. Now 450mg of medicine is present in 500mL of NS, so 1 mg will be present in 500/450mL or 1.11mL and hence 0.359mg will be present in 1.11 X 0.359 = 0.398 or Approx 0.4mL of the solution.
So, the patient should recieve 0.4 ml of the solution per minute. I hope u understood..
