First you need a job offer for a "specialty occupation". Specialty occupations require at least the equivalent of a US bachelor's as entry into the occupation.

Second, you need the right degree/experience related directly to the occupation you will be entering (ex. Computer Science degree for a Computer Programming occupation).

Third, you need a lawyer to put together the petition and supporting documents.

Fourth, lawyer sends package to USCIS with signatures and fee checks from employer.

Fifth, upon approval, you make an appointment at the local consulate and have an interview.

Sixth, get on the plane.

you first need to have an employer to sponsor you, once you have that.... go to the US embassy in your country and apply for the visa (if yours is like mine, you make an appointment, then pay, then go)
or if u are in the us, get your application for done, and mail it in, or go to the nearest immigration office

You find an employer in the US to petition for you. Once the petition is approved, you apply for the visa.
