Survival of the fittest. If a gene ensures that a species will survive and is useful when competing for food, the mutation will become useful and eventually the gene pool of that population may change so that the mutation becomes normal.

If a mutation disables the organism from surviving and producing offspring, however, the organism will die unless it finds a new niche where it is useful. It all depends on the relationship between the mutation and the environment.

Most mutations- as in changes- actually appear first to be defects. There is no way to really answer this without knowing the nature of the 'defect'.
