If he is an unneutered male cat & has big 'cheeks', these are called shields & are something tomcats develop. When male cats fight, they claw at each others faces & these can get scars & get infections, which fester & then burst - not pleasant!

Cats and people have lymph nodes in their necks below the ears. Swollen lymph nodes can be a sign of an infection in the body.

If that area is swollen only on one side that would more likely indicate an infection in the ear. Lower down toward the neck might be swelling from an infected tooth.

My vets always feel the lymph nodes in my cats' necks when they do an exam.

So if there is any infection anywhere a vet needs to examine him and probably prescribe some anitbiotic. The vet will do a blood panel to determine the white blood cell count.

It could have ear mites and the lumps are probably from scratching look in the cats ears and see if there is a wax like substance in them that is dark in color and has a odor. You will need to get ear drops from the vet if this is what the problem is.

If the swelling is only on one side, it may be an abcessed tooth, or a cyst.

If on both sides, only a vet can tell you for sure. Watch him to see if there is any change in his demeanor, eating habits, litter box habits - or if you notice any other unusual behaviour. Having all of this information will help your vet to do a more accurate diagnosis.
