Like how do you get it? by not warming up or lifting heavier weights? and can you prevent it by warming up?

Most common factors leading to muscle tears are: not warming up, not lifting properly, and lifting too much weight. To prevent muscle tearing, always start each exercise with a warm up set. This gets your blood flowing and wakes the muscle up. Lift properly. You see guys swinging their arms and jerking those dumbbells up all the time. Screw that. That only promotes injury and really doesn't do anything for you. Learn how to do each exercise properly, then use the right amount of weight. Lifting heavy is essential to building muscle, but you need to know your limits. I got overzealous one day and strained my shoulder trying to do too much on a military press. I'll never do that again.

Always warm up first. Muscles -- like anything else -- tear when they are subjected to sudden stress.

And start with appropriate size weights/reps. Work your way up to the big stuff ;-)
