Being nuts, or a nutcase.

Someone picked you up and stepped on thee.

Crack-brained comes from the idea of someone being crazy or retarded from suffering a fall and damage to the head or brain; I suppose the "cracked nut" is the head.

During a training course I learnt that the saying came from the name of the road where an institution was Knutford Road or Avenue, hence the name nutter or nut case.

Nut is an old slang term for the head. Many languages have them and the French and Italian words for head derive from the Roman slang word for head (testa) meaning cooking pot. Starting with nut as a term, it is easy to see how someone who was off his head (nut) could be described as a nutter.
