I would like to file a complaint against the dialysis clinic at 105 Tradd Street. How would I do that?

The administrators and employees are uncommitted,careless and rude.They view our lives as just another job.They act as if they are working in Wal Mart or in BiLo.The clinic is also filthy. There is always water and sometimes blood on the floor.They do not clean the chairs the way they are supposed to because there is always blood still left on them. The social worker can never help when everyone knows he can.And he is hardly ever able. How do I go about getting these investigated? Who do I talk to?
