High does of acetaminophen (Tylenol) are bad all by themselves. I had a doctor put me on a medication with acetaminophen in it and I ended up getting toxic from it. Thank God my pharmacist caught it and told me to have my liver enzymes were elevated. I got lucky that it was caught in time.
Since alcohol goes through the liver as well as acetaminophen, I definitely would not mix them. It could ruin you liver or even kill you.

Long-time alcohol users who take acetaminophen in therapeutic or modestly excessive doses are at risk for severe hepatic injury and possibly acute liver failure.
