You should have been instructed that if symptoms become worse you need to contact your physician as soon as possible. The same applies if you have not improved. Strep if not properly treated can cause serious damage to organs like the heart and kidneys.
Zithromax is an excellent antibiotic, but unfortunately some strains of bacteria are resistant to it. That means switching to another antibiotic to achieve a cure. Other cases of strep may still cause symptoms when the medication is all taken but getting worse indicates a treatment failure.
Call your doctor so that you can get an effective treatment. You should also complain if you were not instructed properly in follow up if the medication did not work.
And while we are at it, never stop taking a medication when you feel better (unless a doctor tells you to) because this is one way infections become resistant and more dangerous to us all. Take the whole course every time. If you have bad side effects or if your condition persists or worsens, you should always report this.

Zithromicin continues to be active for a week after you finish it. If you are not getting better, you might need a different antibiotic, because you should have already noticed an improvement. Your doctor might call in a new Rx without seeing you again for another office visit. Pen VK is very effective for strep.
Strep throat can lead to to Rheumatic Fever which usually damages the heart and can kill you if left untreated. If you start getting a red rash on your body call your doctor immediately. If your body starts aching and your joints hurt, you need to be call your doctor.

Z packs are extremely strong, and you should be feeling much better by this time. So, if you are getting worse you need to go back to the doctor. You could actually have strep pneumonia, scarlet fever, or rheumatic fever. Hope you feel better soon!
