It's a simple math problem:
Your goal is a butterfat content of 12%
You know that heavy cream is 36% BF
1% milk is self explanatory
Equal parts give you an average BF content of 18 %
Answer: 2parts 1% + 1 part heavy cream

Substitutions: Equal parts milk and cream
1 cup half and half = 1 cup less 2 tablespoons milk plus 1 1/2 tablespoons melted butter

Depending on what you are making I would see no reason why it wouldn't work. If it is a dessert, I would consider just using the whipping cream and making it especially rich!

Yes -- but since the mixture will not have been homogenized, the cream will separate and float to the top a few minutes after mixing.
