This is an easy one! A malt has added malt powder used for flavoring. Other than that the ingredients are the same. Milk, ice cream, and whatever else you throw in there.
malt is a milk shake with malt powder added to it. The powder is made from extracts of malted barley. They are quite good. It's not easy to find the malt powder anymore, but there are still some places that sell the drink. If you haven't tried one, give it a try. I think you'll like them. My favorite is a chocolate malt

I worked at at a DQ when i was a teen and the only real difference is that we put a teaspoon or 2 of malt in a malt and none in a shake.

Malts taste much better. They have malt powder in them. Some people don't like it though. I have chocolate malt Ovaltine mix at home and my kids hate it. They would rather have plain chocolate instead.

Malts are thicker. I can always tell when they give me a shake & I give it right back to them. My mom got me hooked on them when I was younger.
